New Patients
Welcome to EOC! If your physician has ordered a speech-language pathology evaluation, please call us at 832-632-8813 or email us at info@eoctherapyservices.com to schedule your appointment. Prior to booking your appointment, EOC will need:

• A copy of your insurance card
• Your past medical history/ a recent physician visit note
•Your physician referral form (typically faxed directly to us by your physician).
If you are not being referred by a physician, call us today to discuss your concerns. We can then help you contact your physician to obtain a referral for an evaluation, if deemed appropriate. An appointment for a full evaluation will be scheduled at that time when a referral and/or the necessary documentation is received.
EOC is in-network with Medicaid and most private insurance plans. Contact us to verify. We also accept private pay clients.
Physician Referrals
To refer a patient for an evaluation and treatment for speech-language, cognition, pragmatics, swallowing or voice, please fax a prescription to 832-631-8814 or email it to info@eoctherapyservices.com along with the patient's insurance card and medical history. EOC will then contact the patient directly to arrange an appointment.
EOC will send the referring physician a copy of the patient's evaluation and plan of care, along with periodic updates about the patient's progress. Feel free to call the EOC office at 832-631-8813 with any specific questions, and we are happy to help.
Online Booking
Essence of Communication Therapy Services uses SimplePractice for convenient online booking, document uploads and other services. When new patients are onboarded into our care, we will provide you with login information.
